2014 Columbia University Commencement Ticket Distribution Proxy Form for Columbia College and Fu Foundation School of Engineering and Applied Science (Undergraduates) This form is for graduating CC/SEAS seniors who are not available during the University Commencement ticket distribution times listed below. 1. Print this form, and fill it out completely. 2. Give it to the friend who has his or her own valid Columbia University ID card to act as your proxy during University Commencement ticket distribution. 3. Make sure your friend/proxy brings the following to be eligible to pick up your tickets: a. his or her own valid Columbia University ID card b. this signed proxy statement Eligible CC/SEAS students include October 2013 graduates who did not process in the May 2013 ceremony, February 2014 graduates, and expected May and October 2014 graduates.
