Administrative Success Bookkeeping Fee Sheet Review the service packages listed below to determine which fits your needs. Each method includes the same basic reconciliation services but vary in price. Based on the schedule selected, we will pickup your account statements and will reconcile those accounts. Any payroll functions or filing of reports with outside agencies (Fed, State, Banks, etc) are charged at a rate detailed on the fee sheet. MONTHLY BOOKKEEPING Fee: $50 mo Ideal Client: Business owners tracking business performance on a monthly basis. Also ideal for businesses required to provide regular financial reports to third parties (i.e. investors, banks, etc). 1) Monthly accounting of all business expenses 2) Monthly accounting of sales and deposits 3) Monthly bank reconciliations of business bank accounts 4) Financial reports on as-needed basis QUARTERLY BOOKKEEPING Fee: $250 quarter ($75 mo) Ideal Client: Business owners track their business performance on a quarterly basis. 1) Quarterly accounting of all business expenses 2) Quarterly accounting of sales and deposits 3) Quarterly bank reconciliations of Monthly bank statements 4) Financial reports on as-needed basis ANNUAL BOOKKEEPING Fee: $1200 year (100mo) Ideal Client: Business owners who only to track their business performance on a yearly basis. This is typically a service for Schedule C Federal Income tax filers (with no employees) who have saved all the receipts and need help at the end of the year in preparation for tax-time. 1) Accounting of all business expenses for the entire year. 2) Accounting of sales and deposits for entire year. 3) Bank reconciliations of business bank accounts for the entire year. 4) Financial reports on as-needed basis

Administrative Success Bookkeeping Fee Sheet REGULAR Bookkeeping Services A sample list of the Regular Bookkeeping Services we provide follows: 1) Input Transactions: We enter all your transactions (and match receipts) into Quickbooks on a weekly basis. We will use accounts that match your needs and applicable IRS Guidelines 2) Accounts Payable: We prepare checks for signature and deliver them to you to sign and mail out 3) Accounts Receivable: We will invoice your customers and manage collection as necessary 4) Payroll: We will manage your payroll and maintain your payroll records. 5) Vendor and Customer Files: We will maintain vendor and customer files for all those you do business with 6) Federal & State Payroll and/or Sales Tax Reporting 7) Reconciliation of Bank Statements 8) Financial Statements: We will provide monthly financial statements for your review as part of our basic bookkeeping services package 9) Year End Reports for Your Accountant: We will provide your accountant with the information they need to quickly and inexpensively prepare your taxes if you do not have tax services with our company 10) Year End W-2’s/1099’s STANDARD Contract Bookkeeping Services Fee: $250 month Ideal Client: Business owners who wish to have bookkeeping services but do not require in-office services. Weekly we will conduct bookkeeping duties at our office for delivery to the business when complete. The services provided are no different than the PREMIUM service, they are simply conducted at our office. PREMIUM In-Office Bookkeeping Services Fee: $500.00 Monthly Ideal Client: Business owners who prefer to have STANDARD bookkeeping services happen in-office. Weekly, we will come to your office and conduct bookkeeping duties. We will work with your existing office systems to conduct these services. PREMIUM Bookkeeping Services is essentially the same as STANDARD Bookkeeping Services except we work in YOUR office rather than our own. All functions will be done at your location.

Enter all monthly accounts receivables into computer 4. Provide customer invoicing and statements 5. Balance your books to your bank's monthly statement 6. Provide you with company financial reports as needed $25 Payroll Payroll (One time setup fee $50.00 for 25 or less employees) Run payroll according to established schedules Provide online payroll records to employees (if desired) Complete and efile/mail out all quarterly reports Complete and efile/mail out all yearly reports Process Annual W-2's for employee's Additional Payroll Fees Up to 25 Employees included Each additional 5 employee $10.00 per month Setup fee $50 one time for up to 25 employees OTHER FUNCTIONS $25 Sales Tax (per reporting period) $25 Reports to State/Federal Agencies (per report), other than standard payroll reports

I agree that I have reviewed this fee sheet and agree to the services noted. I understand that this agreement is contracted on a month-to-month basis and can be cancelled at any time. I also understand that services in any month are not prorated based on cancellation during the month. I understand that Administrative Success is not responsible for substantiating income/expenses I submit to them, but are only expected to record such income/expenses.
