2016 HAVIN Insight Expo Registration Form The 2016 Insight Expo, sponsored by the Houston Area Visually Impaired Network (HAVIN), is now set and the only thing missing is you! This will be a half-day event with a focus on Advocacy In Action workshops for the visually impaired. Please pre-register, because it not only saves you money, but it also enters you into a special door prize drawing! Breakfast, workshops, discussion panels, and door prizes, all for only $5! Please complete all information on the accompanying form and submit third page of the registration packet, along with payment for all who will attend. Bring the signed Media Release form with you to the event. Pre-registration Deadline: July 15, 2016. (All entries submitted after this date will pay the full price of $10 per person) Pre-registration fee is $5 per person. If you wish, you can register on the day of the event, but without the pre-registration discount, the cost will be $10 per person. If you would rather send this form in by email and mail in only the payment, that would be fine. However, make sure to note the party you are registering in the memo field of your check. University of Houston – Main Campus Health and Biomedical Sciences Building (Marked UHCO on MAP) 4811 Calhoun Houston, Texas 77004 Traveling northbound on I-45: Exit Elgin-Lockwood/Cullen. Turn left on to Elgin. Travel to the 2nd light and turn left on to SPUR 5. Travel to the 1st light and turn right on to University Drive. Travel to the next light and turn left on to Calhoun. Entrance (#2) to the Patient & Visitors Parking Lot will be located on the left, past the Wellness Center and between the buildings marked UHCO on the map.
Traveling southbound on I-45:
Exit SPUR 5 south. Travel to the 1st light and turn right on to University Drive. Travel to the next light and turn left on to Calhoun. Entrance (#2) to the Patient & Visitors Parking Lot will be located on the left, past the Wellness Center and between the buildings marked UHCO on the map.
Overflow Parking is available across the street in the Welcome Center Garage. Visitor parking is on the 3rd floor and up. The weekend parking rate is $3.
