SUMMARY OF COMMUNICATION PROTOCOL FOR DIAGNOSED CONCUSSIONS PARENTS/GUARDIANS Parents/Guardians should complete STUDENT TRACKING FOR CONCUSSION MANAGEMENT form and share the following information with the school as soon as possible: 1. Student name, grade, gender; 2. Mechanism of injury (this should also include sport, location, date and time); 3. Confirmed diagnosis (this should also include the name of physician and date of diagnosis); 4. Anticipated duration of absence (this should be based on recommendations from diagnosis).

SCHOOL RECEPTION Upon receiving a completed STUDENT TRACKING FOR CONCUSSION MANAGEMENT form, the Principal/designate will: 1. Maintain a Concussion Management File to be kept with the student’s OSR, with name, grade, gender, mechanism of injury, details of diagnosis, and anticipated duration of absence) 2. Inform all appropriate staff members of the concussion (i.e., teachers, coaches, guidance, administration, student success, student services, and athletics staff) 3. Provide parents/guardians with the Concussion Management Protocol and concussion management resources;

INITIAL STUDENT RETURN TO SCHOOL Upon returning to school for the first time after sustaining a concussion-related injury, the following actions are taken: 1. Principal or Designate notifies all appropriate staff members of student return 2. An In-School Team Meeting is scheduled to review CMP and to provide additional supports if necessary.

APPOINTMENT WITH PRINCIPAL, DESIGNATE OR IN-SCHOOL TEAM MEMBER(S) During the appointment with the returning student: 1. 2. 3. 4. 5.

Review the Concussion Management Protocol with the student; Share resources and supports Communicate with classroom teachers Communicate with parents/guardians to share the support plan, request parental input, and review the CMP protocol; Initiate the Summary of Accommodations sheet for individualized in-class accommodations based on student needs.

CLASSROOM TEACHERS When a student returns to the classroom, teachers will: 1. 2. 3. 4.

Refer to Concussion Management Protocol; Conference with the student to implement a support plan in accordance with CMP guidelines; Communicate with parents /guardians to share academic plans and supports; Maintain communication with appropriate staff members (i.e., student success, special education, guidance departments) as needed.
