Parent Showcase will begin promptly at 2:00pm in the CHS Gym

I understand that participation in this athletic program is strictly voluntary and I freely chose my child to participate. I verify that I will be responsible for any medical costs my child might incur as a result of participation. I am aware that camp will not begin until 9am that morning and that there will be no adult supervision for my child until that time. I will be responsible for myself or the selected party who signs my child in and out of camp.
