Use this form for the following processes: C22, E2, E4, C41 films more than 10 yrs old, 126 film or 110 film from 1970-1990, all Panchromatic films, Vpan, Verichrome Pan, Kodachrome Slide Films, Unknown process films (unlabeled), aged black and white films. The following sizes of films are serviced: 120, 620, 127, 828, 122, 116, 616, 126 cartridge, 110 cartridge, 4x5 Sheet Film and others. For other unknown films or processes, use this form. Service Pricing Develop and Recovery Cost: $29.95 developing per film PLUS .95 PER IMAGE recovered/printed. The following 4 sizes of films are $5.00 additional service price – 116, 616, 122, 130. Roll Films (120, 620, 116, 616, 828, 127, 122) are estimated at 12 images maximum for prepay purposes. Other films are estimated at the number of exposures listed on the film cartridge or container. All develop and print services include a FREE CD of recovered images. CD is not included with the develop only option below. Developing and shipping/handling charges are non-refundable regardless of results. Refunds for prepaid unrecoverable images are refunded at the end of the order when the order is shipped to customer. This is a PREPAY SERVICE ONLY. If paying by credit card, your card is charged the amount of the estimated order total recovery before processing is started. After processing is complete, all
credits are given at the time of shipping. This specialized service takes 5-10 weeks following payment of your order. By submitting this form with payment or signing form, you agree to pay the appropriate amount due on your order and understand the developing fee is not refundable regardless of results. Orders received without approved prepayment will be returned via uninsured regular mail. Limit of Liability - Submitting any film to this firm for processing, printing or other handling constitutes an agreement by you that any damage or loss by our company, subsidiary, or agents, even though due to the negligence or other fault of our company, subsidiary, or agents, will ontly entitle you to replacement with a like amount of unexposed film. Except for such replacement, the acceptance by our company, subsidiary and/or agents of the film is without other warranty or liability, and recovery for any incidental or consequential damages is excluded. Submitting any film for processing or handling constitutes an agreement that this firm may use recovered images in limited promotion and display without further compensation unless a written note is included on the back of this form stating to not use recovered images for promotion or display.

Then include the order form plus film and payment in a bubble pack or box and ship to address at top of form. For RUSH ORDERS, mark the outside of the shipping container “RUSH” SHIPPING AND HANDLING CHARGES (Shipped USPS w/confirmation) First Film, $7.95 plus .60 for each additional film enclosed (USA). For International, add $20.00 to US shipping costs above. Shipped via registered insured mail only.
