 Position Level Descriptor An individual at the Team Member level is accountable for their own performance and contributes to team performance. People at the Team Member level do not have any people reporting to them on a day to day basis.

 Position Summary The Street to Home Case Manager will provide relationship based intensive case work and conduct assertive outreach to support people who are sleeping rough, or experiencing chronic homelessness to move into stable, long term housing. Support will focus on achieving long term lifestyle changes to enable engagement in the community, education or employment where appropriate. It is envisaged that the service will: -

Undertake assertive outreach to identify and support people who are rough sleeping and people experiencing chronic homelessness (i.e. parks, squats, shelters).

Use a ‘housing first’ approach to work with the client to move from rough sleeping or chronic homelessness into appropriate stable housing.

Provide intensive case coordination to assist the client to remain in long term stable housing.

Link clients with other ongoing support as appropriate.

The position will be required to work after hours on a regular basis.

 Position Responsibilities Key Responsibilities 

Provide relationship based intensive casework and conduct assertive outreach.



Attend team and case work meetings to share information with other caseworkers to maintain a consistent approach.



Participate in assertive outreach activities and facilitate social and wellbeing programs.



Develop and maintain a case plan cooperatively and transparently with each client.



Use brokerage funding as per the case plan, when the most appropriate service can not be sourced through free referral.



Maintain accurate and up to date data collection and reports.



Ensure service delivery is in line with the national strategy and funding agreement.

 Position Selection Criteria Technical Competencies  Ability to work directly with homeless clients with complex needs from a variety of cultural backgrounds, to achieve positive sustainable outcomes.  Demonstrated knowledge of casework principles with proven experience in casework services for vulnerable ‘at risk’ people.  Highly developed oral and written communication skills.  Ability to work effectively and collaboratively as part of a team.  Ability to build relationships with a variety of people across different community organisations, sectors and government departments.  Sound proficiency in MS Office and client databases.

Qualifications/Licenses  Tertiary qualifications in social work, community services, or human services or related field and/or  Experience in the human services sector or a related field.  Current open driver's licence.  Working with Children Blue Card, or the ability to obtain one.
