DO-IT-YOURSELF FUNDRAISING Thank you for your interest in choosing the Epilepsy Foundation (EF) as the recipient of your fundraising event! Do- ItYourself (DIY) fundraising events assist in funding our many programs and services that will benefit individuals with epilepsy, their families and their caregivers. POLICY & PROCEDURE AGREEMENT Please complete and sign the Epilepsy Foundation’s DIY Event Application and Agreement sheet, which describes the conditions under which a third-party event can be held using The Epilepsy Foundation’s name and/or logo. A staff member will review the agreement, and provide approval for your activity/event. The third-party event is not authorized until you receive an approval. If you are under the age of 18 your parent or guardian must sign the third party application and agreement sheet on your behalf. EPILEPSY FOUNDATION REPRESENTATIVE The Epilepsy Foundation does not provide staffing for third-party events with respect to registration, collection of funds, distribution of materials, etc. If you would like to have a representative at your event, we will make our best effort to have a volunteer attend to accept a check and/or to address the attendees. Because of the large number of events staff members are generally unable to attend thirdparty events. MATERIALS & RESOURCES The Epilepsy Foundation will provide materials about the Epilepsy Foundation which the organizers can print to be distributed at the event at the organizers’ request. The Epilepsy Foundation authorizes the Third Party use of its name and logo in communications to the media upon obtaining prior approval from the Epilepsy Foundation DONATION OF PROCEEDS Organizers of the event donate the net proceeds of the event to the Epilepsy Foundation. Donations may be collected and submitted two ways. Check(s) donations during the event can be made by writing a check directly to the Epilepsy Foundation, with the name of the event on the memo line, then mailed to Epilepsy Foundation, ATTN: DIY, 8301 Professional Place East, Landover, MD 20785. Please send within 10 days of the event with the donation form. EF provides a single acknowledgment for the event proceeds; it does not acknowledge the contributions of individual event participants. Credit or Debit Card through our online fundraising tool. Steps for setting up your fundraising page will be emailed to you after your event has been approved. Note that The Epilepsy Foundation does not pay any costs associated with the event. COST The Epilepsy Foundation will not cover expenses or assume any legal or financial liability associated with the DIY Event. Organizers of the event cannot use the Epilepsy Foundation’s tax exemption in conjunction with the event.
