Memorial Oaks Paver Request Form I want to support the Memorial Oaks of South Kenton project by purchasing an engraved paver. I understand that I can respectfully honor any individual, family, or group with a paver. To honor an individual teacher, school administrator, Alumni Class, or anyone else, select the appropriate sized paver and complete your message in the form below. To honor an armed forces hero, we suggest you include: Name; Rank; Service Branch (USA, USN, USMC, USAF, USCG, USMS) and Era/Time Frame (WW II, Korea, 1978 - 1982, etc.).
To honor a first responder hero, we suggest you include: Name; Position; Unit; and Time Frame.
