National Passport
Travelers must file a visa application in the Peruvian Diplomatic or Consular Mission with the following accompanying documents: • Form DGC-5 de visa application • A copy of the identification page from a Valid Passport, with minimum validity of six months • One color photograph, facing forward, with a white background, including (uncovered) head shoulders. Size of photo must be 2 inches x 2 inches or 51 mm x 51 mm. • A copy of the Annual Meetings Registration Confirmation Letter
Delegates, Staff, Observers, Guests, CSOs, Speakers, general Participants and their Family Members will be on OFFICIAL VISA at No Fee, upon presentation of Annual Meetings Registration Confirmation Letter.

Participants must upload the following documents: • A copy of the identification page from a Valid Passport, with minimum validity of six months • Form DGC-5 de visa application • One color photograph, facing forward, with a white background, including (uncovered) head shoulders. File size must be less than 500 Kb. • A copy of the Annual Meetings Registration Confirmation Letter • After verification all requirements have been fulfilled, The Ministry of Foreign Affairs will issue an “Official e-Visa”, to allow applicants to board their flights.
