This form, along with the appropriate supporting financial documentation must be completed and submitted to the ISSS office. The I-20 form will not be issued until this form and all supporting documentation is received . Please note that you will also be required to present supporting financial documents at the U.S. Embassy/Consulate at the time of visa interview and/or at the port of entry.
