Programs at Camp Boggy Creek include week long summer sessions as well as family retreat weekends and each includes full medical coverage with on-site professionals in the illness specialty being served. Each group has set dates to attend. When and where to find applications: When applications are available: • Summer(camper only)- January 1st • Family Retreat, Fall / Winter- July 15th • Family Retreat, Winter/ Spring- November 15th Summer Camper with the diagnosis ages are 7 through the age of 16. This is both developmentally and chronologically. Retreat Camper with the diagnosis ages are 5 through the age of 16. This is both developmentally and chronologically. To complete or create an account: • If you have an account or if your child has been before but are unsure of your log in information, click on ‘forgot log in information’. If you are using the same email that we were given, the system will look you up by that email. • If this is your first time applying you will create an account. • To create an account o Add in Parent contact information first o Once you complete Parent / Guardian information you will see below your name to the right‘Add person to this account’ click on this o Now enter the child with the diagnosis and the rest of the family members, be sure to save after each family member is added (Grandparents are welcome to join you for the Family Retreats but please do not include aunts, uncles, or cousins or siblings that have their own family, even if they live with you) o If applying for summer camp you will make an individual reservation under the camper’s name. If applying for the family retreat select small group o Select the session with your Childs diagnosis

o Just a few pages away from being done. The last page of the application for summer is the

electronic ‘Mandatory Waiver Consent’, for the retreats the last page is the ‘Parent / Guardian consent’. After completion of the application it will automatically come to us. You will receive an email letting you know application has been received (this does not guarantee acceptance, this is just the start of the application process). Attached to that email will be the medical forms that your child’s specialist is to complete. Don’t forget to request a copy of the most recent office notes from your child’s specialist. It is your responsibility to be sure all requested medical information has been turned into Camp Boggy Creek. Incomplete applications cannot be considered.

Once we receive the medial portion of the application we can start the review process. We do not select on a first come first serve basis. Summer applicants will be notified on their status as follows: • Weeks 1 and 2, April 10th • Weeks 3 and 4, April 17th • Weeks 5 and 6, May 8th • Weeks 7 and 8, May 15th Family Retreat applicants will be notified on their status • One month prior to the retreat start date
