ABOUT YOUR COURSE INSTRUCTOR Mr. Kolbeck received his Bachelor of Science in Physical Therapy from the University of Wisconsin—La Crosse in 1991 and has practiced in the orthopedic and sports medicine setting his entire career. He is dual board certified in Orthopedic and Sports Clinical Specialties, and is a Certified Orthopedic Manipulative Therapist (COMT) Level IV graduate of the North American Institute of Orthopedic Manual Therapy (NAIOMT), as well as graduated from their fellowship program. In addition, Karl has been trained both as a Clinical Fellowship and Faculty Instructor with NAIOMT and is a past faculty member with NAIOMT. Thereafter he founded and operated Empiridence® Seminars. In 2005 Karl became a Fellow of the American Academy of Orthopedic Manual Physical Therapists (FAAOMPT).

Karl is co-owner of Rose City Physical Therapy in Portland, OR with specialization in the shoulder girdle and running athletes. He works with Team USA professional rock climbers and bouldering athletes, the Portland rock climbing community, and multiple high school, regional, collegiate and national level swimmers, tennis players and pitchers with shoulder related problems. He is also a member of the medical team for the Bowerman Track Club - a Portland based Nike professional running team with multiple international World and Olympic team athletes. He enjoys the complex patient and the challenges that come with treating those cases.

Karl’s career has been spent studying and treating the orthopedic and sports injured shoulder and the relationship it has with the cervical and upper thoracic spine. He has been instructing orthopedic and sports medicine physical therapy and manual therapy courses since 2001. In 2005 he founded and was President of Empiridence® Seminars until 2015 when he ceased Empiridence® Seminars operations to venture into private practice. He continues to teach by invitation. He has taught nationally and enjoys sharing his knowledge and clinical experience by providing a comprehensive integrated approach to clinical evaluation and management of the shoulder girdle based on empirical knowledge and evidence enhanced practice.

COURSE DESCRIPTION

COURSE OBJECTIVES

Advanced Shoulder: Integration with the Cervical & Thoracic Spine is a 2-day lecture and lab based course that provides a sound biomechanical examination and treatment model for the cervical and upper thoracic spine as well as the upper ribs as they relate to the shoulder girdle. Information is presented in a clinically applicable format for immediate integration into clinical practice. This course develops advanced skills for effective differential diagnosis and “best” evidence based management strategies for the cervical and upper thoracic spines and the upper costal region for treatment of the complex shoulder patient. Safety measures and safety clearance assessment as well as indications for and contraindications against thoracic spine grade 5 HVLAT (high-velocity low-amplitude thrust) are also offered. Hands-on manual therapy treatment will be the main component of this course, including advanced manual therapy skills (soft tissue augmentation, indirect positional release, muscle energy techniques, spinal joint mobilization & grade 5 HVLAT), postural correction and patient education.

• Understand evidence based implications due to postural dysfunction (kyphosis, forward head, spinal stiffness) on the scapula, the supraspinatus outlet and GH joint function • Prescribe postural exercises and apply empiridence based corrective therapeutic exercise including respiratory facilitated postural correction and foam roller exercises • Define spinal “Segmental Facilitation” and understand its implications on the shoulder girdle • Evaluate to rule in/out spinal “Segmental Facilitation” influences on the shoulder girdle • Describe influences that cervical, upper thoracic and 1st & 2nd rib dysfunctions have on the shoulder girdle • Apply empiridence based evaluation and treatment techniques for the cervical, upper thoracic and 1st & 2nd ribs • Define the principles of spinal grade 5 HVLAT techniques, including safety and risk clearance assessment, and indications for and contraindications against grade 5 HVLAT to the CT junction, T1-2, T2-3 and ribs 1 and 2 • Apply soft tissue release techniques, indirect and direct muscle energy assisted mobilizations, joint mobilizations and grade 5 HVLAT techniques
