Entry Form Facilities Management Team Award Most facilities management teams focus on multidisciplinary activities, combining resources and activities to deliver a safe and efficient work environment. This award recognises the achievements of the facilities management (FM) team, whether in-house or out-sourced, who delivers outstanding building performance from an individual building, a site with several buildings or a portfolio of assets. This will include delivering the comfort levels and working conditions required by the users, whilst demonstrating substantially reduced carbon emissions, energy and water consumption. Entries may be submitted by any or all members of the FM team, for a building or buildings either within the UK or overseas and should cover a full year of facilities management activity, supported by operational performance data, during the period 1 June 2014 - 31 August 2016. Please complete the entry form below. The headings reflect the judging criteria and the judges will be looking for you to provide the relevant information under each heading.

Entry criteria Please outline how your entry meets each of the criteria. Judges will be looking for information in each of the sections when assessing the entries. Entries which do not clearly address all the criteria are unlikely to score highly. Any documents, charts or photos should be referenced and included in your supporting documents. The word count for each criteria is not prescriptive and acts only as a guide.

1. Please provide a synopsis of the energy management initiative or strategy and its building performance objectives, focusing on low carbon, energy efficiency and the health and wellbeing of occupants. Word count guide: 200 – 300 words

2. Provide details of the building(s) or portfolio and its performance requirements. Word count guide: 200 – 300 words

3. Provide evidence of specific facilities management measures taken to improve building performance including changes to the lighting, heating and cooling provision, changes to equipment, hours of operation or other control parameters. Word count guide: 300-500 words

4. Provide evidence of improvement in actual performance based on measured energy and water consumption, for example:  a copy of any DECs and associated information or ESOS report  environmental, sustainability, carbon or utilities reports; or CarbonBuzz entries Word count guide: 300-500 words

5. Provide evidence of user feedback on the building(s) or portfolio and satisfaction with the measures adopted. Word count guide: 300-500 words

6. Provide evidence of collaboration between members of the facilities management team and users that has contributed to improved performance. Word count guide: 300-500 words
