Entry Form Energy Management Initiative Award The award recognises the initiatives of a building owner or occupier seeking to improve their energy management and achieve outstanding building performance in their building or building portfolio. This will include demonstrating substantially reduced energy consumption and carbon emissions without compromising the comfort levels and working conditions of its users. It will be awarded to the organisation who demonstrates the most effective initiative and strategy for managing energy and water use and makes the most efficient use of the energy it does consume. Entries may be submitted by any organisation for an initiative either in the UK or overseas, where the initial activity was completed during the period 1 June 2016 - 31 August 2017. Entries should be accompanied by a full year of operational performance data following completion of the initial energy management interventions. Please complete the entry form below. The headings reflect the judging criteria and the judges will be looking for you to provide the relevant information under each heading.

Judges will be looking for information in each of the sections when assessing the entries. Entries which do not clearly address all the criteria are unlikely to score highly. 1. Please provide a synopsis of the energy management initiative or strategy and its building performance objectives, focusing on low carbon, energy efficiency and the health and wellbeing of occupants. Word count guide: 200 – 300 words

2. Provide details of the building(s) or portfolio and its performance requirements. Word count guide: 100-200 words

3. Provide details of the energy management initiative or strategy. Word count guide: 300-500 words

4. Provide evidence of the measures taken to reduce energy use, e.g. changes to the lighting, heating, and cooling provision, changes to equipment, hours of operation or other control parameters; changing staff or user behaviour by training, education and motivational activities. 5. Show evidence of improvement in actual performance based on measured energy and water consumption, for example: • a copy of any DECs and associated information or ESOS report • environmental, sustainability, carbon or utilities reports; or CarbonBuzz entries Word count guide: 300-500 words

6. Provide evidence of user feedback on the building(s) or porfolio and satisfaction with the measures adopted. Word count guide: 300-500 words
