Evan E. Worthing Scholarship Overview This year, the Evan E. Worthing Scholarship Program (through Bank of America) is committed to providing four year scholarships of $2,000 per year to fifteen (15) graduating seniors within the Houston Independent School District (HISD).

Eligibility Requirements 1. A minimum GPA of 3.25 is required. 2. Attend the Worthing Scholarship Presentation and Reception June 2, 2016 4:00p.m. p.m.‐ May 31, 2017from at 2:00 at 6:00 Young Branch Libraryto (5107 Griggs Rd. Houston, TX 77021) in Conference Room for Dr. Teddy McDavid. p.m. (location be announced). 3. Complete all requirements for graduation by May 26, 25, 2016. 2017. 4. Attendance at the Worthing Scholarship Summer Institute June 2016 is required. The June 6‐10, 5‐ 9, 2017 Summer Institute will be held at DeBakey High School from 8:30 a.m. to 1:45 p.m. and will cover information and skills needed to help students make the successful transition from high school college level. to university level. 5. Failure to enroll into an accredited college or university by Fall 2016 2017 will result in forfeiture of the scholarship.

Application Instructions Students should complete the application electronically using the fillable PDF in Naviance, print it, attach all required documents, and submit to their Counselor or Principal before the required deadline. No handwritten applications will be accepted.

Required documents to be submitted with application 1. An essay of no more than 100 words describing how this scholarship would provide an educational opportunity for you. 2. A letter of acceptance from the accredited college or university to which you are expected to attend. 3. A letter of recommendation from a school official who can indicate substantial knowledge of your character, activities, and commitment to pursue higher education. 4. An official high school transcript including score report for either SAT or ACT with official school seal in a sealed envelope. 5. A clear portrait style photo for identification and media purposes.
