JOIN US AT THE 2015 NAPLES NKF GOLF CLASSIC The 2015 Naples NKF Golf Classic founded by DeVoe Cadillac is proud to welcome back The Franklin Templeton Shootout as this year’s presenting sponsor. The event will be held on Sunday, December 6, 2015 on the Championship Gold Course at Tiburón Golf Club at The Ritz-Carlton Golf Resort, Naples as part of the Franklin Templeton Shootout, a nationally televised PGA TOUR event which will be played at Tiburon later that week. The top three finishing foursomes from the handicap index division of the tournament will receive invitations to participate in the NKF Golf Classic National Finals at the magnificent Pebble Beach Resorts in Monterey, CA with the winning foursome also receiving vouchers for air travel to participate in the finals at Pebble Beach. This year a gross division will be added to the competition and the top three finishing teams in this division will also be invited to the NKF Golf Classic National Finals at Pebble Beach in January 2016. The top team in the gross division will also receive air travel vouchers to the finals at Pebble Beach. All contributions should be made payable to Shark Shootout Charities, Inc. to benefit the National Kidney Foundation of Florida. ABOUT THE NATIONAL KIDNEY FOUNDATION The National Kidney Foundation, a major voluntary nonprofit health organization, is dedicated to preventing kidney and urinary tract diseases, improving the health and well-being of individuals and families affected by kidney disease and increasing the availability of all organs for transplantation One in nine or more than 20 million Americans suffer from kidney disease; most do not even know they are at risk. 95,000 Americans die of renal disease every year. There are more than 95,000 patients waiting for a kidney transplant this year while only 11,000 will receive transplants due to the lack of organ donors. Seventeen people on the transplant waiting list will die every day. Currently the National Kidney Foundation and its Affiliates spend 83% of all revenue generated by these golf events on patient services and programs including organ donation, renal transplantation and direct aid to those patients in need. This Greg Norman-designed course was carved from 800 acres of prime Southwest Florida real estate. However, the course sculpting was limited to 230 acres, preserving as many native areas as possible. Winners in both gross and handicap divisions qualify to compete in the National Finals at Pebble Beach, California in January 2016. However, all handicap division teams must have a combined U.S.G.A. handicap index of 42.0 or more with only one golfer having an index of 8.0 or less in order to qualify. Facts About Kidney Disease 

More than 20 million Americans (1 in 9 adults) have chronic kidney disease. Another 20 million Americans are at increased risk.



High-risk groups include anyone suffering from high blood pressure, diabetes, a family history of kidney disease, older Americans, African-Americans, American Indians, Hispanics, Asians and Pacific Islanders.



High-risk groups should ask their physicians for three routine tests: a blood test for creatinine, urine test for protein and measurement of blood pressure.



Nearly 16 million Americans, or about 6 percent of the U.S. population, have diabetes.



About 50 million Americans (1 out of 5 adults) have high blood pressure.



More than 370,000 Americans are being treated with dialysis or kidney transplantation for kidney failure.



Each year, nearly 95,000 Americans die from causes related to kidney failure.

Regional Statistics... 

1 in 9 Floridians, or 1,800,000 individuals in our state, have chronic kidney disease.



Another 1,800,000 adults in Florida are “at risk” for chronic kidney disease.

Over 95,000 people are waiting for organ transplants nationwide, of those 1,470 are waiting in Florida.

Florida offers more than 12 health screenings each year to identify community members who may be at risk for kidney and urologic disease

USGA Handicap Index

How to Form a Handicap Index Division Team  Any amateur golfer, male or female, is eligible to compete in either divisions in the NKF Golf Classic.  Every team competing in the Handicap Index Division must consist of four amateur golfers whose total USGA Handicap Index is 42.0 or more and only one (1) golfer on each of these teams may have a USGA Handicap Index of 8.0 or less.  Golfer’s handicap indexes are effective as of the latter of: (i.) The day of registration for the tournament (i.e. the day the entry fee is paid) or (ii.) The day sixty days prior to the tournament  Each golfer who has a USGA Handicap Index will be required to present a current copy of his or her handicap card, including GHIN# and name of home course to the tournament committee at the time of registration.  All golfers must present a photo ID on the day of the tournament.  Any team that contains a member who does not comply with these requirements can play in the local tournament, but will be competing in the Gross Division,  The National Kidney Foundation has the right to deny the entry of any players or teams. Team USGA Handicap Index: All handicap disputes shall be settled by arbitration with the USGA (or governing state golf association) in accordance with USGA rules, and the decision rendered by the arbitrator may be entered in any court having jurisdiction thereof. Any golfer who is found to have misrepresented his/her USGA Handicap Index shall be disqualified. The National Kidney Foundation reserves the right to report flagrant infractions to the state golf association and USGA. National Kidney Foundation Golf Classic Committee decisions are final and not subject to appeal. This rule does not prevent the committee from correcting an incorrect ruling and imposing or rescinding a penalty
