Health Management Partners Client Story How a health and disease management company distinguished itself in the marketplace by using behavior change science to strengthen its services

The Challenge Health Management Partners (HMP) of South Dakota provides a variety of care extension support services such as wellness programs and condition management. HMP uses Healthwise for Care Management to connect care managers to evidence-based health education seamlessly within their workflow. The company focuses on building relationships and strives to provide compassionate care while improving members’ health and reducing costs.

HMP had two main challenges: ++ To make their programs and care plans consistent across service lines. Care managers were sending health education from various resources to patients, and there were significant discrepancies in how the different programs were designed and the kind of information members received. ++ To improve outcomes and demonstrate their value, while also maintaining their member-centric approach.

Our Solution Healthwise provided the tools for HMP to deliver tailored interventions to their members and worked with the organization to find ways to improve coaching efficiency while motivating healthier behaviors and improving outcomes. Integrate Behavior Change HMP wanted to incorporate behavior change best practices into their services to positively affect members’ health behaviors and create lasting change. As part of the Healthwise for Care Management solution, HMP has access to Healthwise® Behavior Change Navigator, a tool that uses behavior change science to facilitate tailored interventions. Navigator allows coaches to quickly identify a person’s behavior change stage, and then suggests the best coaching tips and health education materials to meet that person’s unique needs. Healthwise trained HMP staff on Navigator and motivational interviewing (MI), including best practices for incorporating them into their calls.

Improve Program Design Healthwise consultants facilitated an onsite session with HMP program leadership staff to review and evaluate the organization’s current care plan designs, processes, and outcomes measures. The consultants identified HMP’s strengths and weaknesses and then presented a report of customized best practices to develop and deploy effective health coaching programs. While onsite, Healthwise also observed the coaching staff during calls to see firsthand how program design translated to member interactions. Together, Healthwise and HMP built a roadmap for program optimization based on these evaluations and observations. The steps identified in the roadmap included: ++ Developing consistent programs and care plans. ++ Integrating MI principles and behavior change science into programs and member encounters. ++ Implementing technology enhancements to improve coaching workflow and efficiency.

Behavior Change Results Healthwise’s approach to behavior change identifies five stages people may go through when working toward healthier behaviors. The support that people need to achieve their goals is different in each stage. As shown in the graph below, people’s stage and needs change over time. Healthwise Behavior Change Navigator helps coaches understand which stage their members are in and suggests appropriate supporting education tailored to the member’s unique needs.
