Haven for Hope of Bexar County WAIVER AND RELEASE OF LIABILITY FORM Activity or Event__3rd Annual Hoops for Hope Basketball Tournament__ Date of Activity or Event____Saturday, March 24th of 2018________ Check One: � Participating � Volunteering In consideration of Haven for Hope of Bexar County and its affiliates and each of their directors, officers, volunteers, representatives, and agents (collectively, “Haven for Hope”) sponsoring and/or permitting me to participate in the Activity or Event identified above, and intending to be legally bound, I, the participant identified above (“Releasor”) and on behalf of my agents, heirs, executors, administrators, and assigns (individually and collectively, the “Releasor Parties”) hereby agree as follows: I HEREBY WILLINGLY ACCEPT AND ASSUME ALL OF THE RISKS OF PARTICIPATING AND/OR VOLUNTEERING IN THIS ACTIVITY OR EVENT AND ANY TRANSPORTATION TO OR FROM OR IN ANY WAY RELATED TO THE ACTVITY OR EVENT, including by way of example and not limitation: any risks that may arise from negligence or carelessness on the part of Haven for Hope; dangerous or defective equipment, property, or facilities, whether or not owned, maintained, or controlled by Haven for Hope; temperature; weather; lack of hydration; vehicular traffic; and actions of other people including, but not limited to, participants, volunteers, spectators, Activity or Event monitors, and/or producers of the Activity or Event. 1. I certify that I am physically fit and have sufficiently prepared for participation in the Activity or Event, and I am not aware of any physical problem or condition that will limit or interfere with my ability to participate in the Activity or Event. 2.

I agree to abide by any applicable Haven for Hope instructions, rules, and regulations.

3. I WAIVE, RELEASE, DISCHARGE, and HOLD HARMLESS Haven for Hope from and against any and all liability of any kind whatsoever that I or the Releasor Parties may have now or in the future against Haven for Hope in any way resulting from, arising out of or relating in any way to the Activity or Event identified above, INCLUDING BUT NOT LIMITED TO, LIABILITY ARISING FROM THE NEGLIGENCE OR FAULT OF HAVEN FOR HOPE, for my death, disability, personal injury, property damage, property theft, or actions of any kind that may hereafter occur to me including in connection with my traveling to and from this Activity or Event. 4. INDEMNITY.
