Applicable Paperwork Proof of ID (Photo ID), Social Security cards for you, your spouse, and dependents, birth dates for you, your spouse and dependents, wage and earning statements (W-2s, 1099’s), interest and dividend statements, copy of last year’s tax returns, proof of bank account routing and account numbers for direct deposit such as a blank check. Both spouses must be present to sign joint return. Total paid to daycare provider and provider’s tax ID number, Affordable Health Care statements (1095-A, B, or C), Copies of Income transcripts from IRS and state, if applicable.
