What’s new? Locality Planning Locality planning leads have now been identified for the four locality areas in South Lanarkshire. The locality lead officers are:    

Clydesdale: Marilyn Aitken (NHS Lanarkshire) Rutherglen/Cambuslang: Tom Bryce (NHS Lanarkshire) Hamilton: Deborah Mackle (South Lanarkshire Council) East Kilbride: Nadia Ait-Hocine (South Lanarkshire Council)

What will these officers do? The officers will have a key role in leading the partnership locality planning agenda, particularly from a strategic commissioning viewpoint. That means they will have a key role in planning how services are delivered in local areas. Crucially, they will ensure the locality plans assist in shaping the wider strategic plan (read more on the strategic plan below). In so doing, they will also ensure close linkage between the Integration Joint Board (IJB) and the Health & Social Care Partnership (H&SCP) management team. Work is underway to establish and plan how the locality lead officers will engage with people at locality level. Details will be published in due course. For more information on locality planning, visit: http://bit.ly/1Oy7CU8 Locality Profiling South Lanarkshire is made up of four localities - Clydesdale, Hamilton, East Kilbride and Rutherglen/Cambuslang. Each locality now has a profile developed that provides a detailed picture of health needs and trends at a local level. It is an evidence-based approach which will inform how plans are shaped and tailored now and in the future. One area of work currently being undertaken is in relation to health-based information regarding people with long-term conditions. We are also focussing on the joining-up of health and social care data to inform the partnership of the number of high resource service users in any given area. This will include, for example, people who are in receipt of both social care and health care.

The partnership is receiving support nationally through the Information Services Division (ISD) of NHS National Services Scotland in the form of dedicated staff to assist with locality profiling. Overall, it is important to note, this work is governed by strict privacy and ethical guidelines. Board established The Order to formally establish the Integration Joint Board (IJB) has now been approved at The Scottish Parliament. What is the background? Last year, The Public Bodies (Joint Working) (Scotland) Act was passed which means changes to the law that require health boards and local authorities to integrate adult health and social care services. To take this reform forward, we have set up the South Lanarkshire Health and Social Care Partnership (H&SCP) which will be responsible for community health services for all age groups, hospital care associated with unplanned admissions and adult social work services. The services will continue to be delivered by the NHS and the local authority social work services. The IJB will provide the strategic overview to ensure plans are delivered by the H&SCP and respective partners. It had its first formal meeting, as a legally established entity, on 6 October 2015. Who is on the IJB? The Board has, as its core membership, eight voting members and a number of non-voting members as defined in the legislation. Councillor Jackie Burns has been appointed Chair of the IJB. The Board members also include Councillors Maureen Devlin, Allan Falconer and Lynsey Hamilton; NHS Board NonExecutive Directors Philip Campbell (Vice Chair), Lilian Macer and Tom Steele and also Medical Director, Dr Iain Wallace. Harry Stevenson took up the post of South Lanarkshire H&SCP Chief Officer on 3 March 2014. Harry will lead the new integration team as it seeks to further improve quality and consistency of care in South Lanarkshire. Key plans The Integration Scheme The integration scheme can be viewed at: http://bit.ly/1MsoeNV The integration scheme sets out how integration will look in South Lanarkshire, including the functions of health and social care which will be delegated to the Health & Social Care Partnership (H&SCP). The Strategic Commissioning Plan The strategic commissioning plan is at the heart of integration and is intended to be the means by which services are delivered in a more integrated way to improve the quality of support for people who need them.

The strategic commissioning plan will set out how health and social care services will be delivered to improve the outcomes of the people of South Lanarkshire. The IJB will put in place the strategic commissioning plan for functions and budgets under its control. It will also set out its plan for the following three years. An early draft outline strategic commissioning plan has been produced and work is now underway to refine this. True to our vision - Working together to improve health and wellbeing in the community - with the community - engagement with the public, staff, stakeholder and public is key to influencing and shaping the plan. Engagement How are people being engaged? The Strategic Commissioning Group has met on two occasions and a suite of meetings have been agreed for the remainder of this financial year. The Group is made up of a broad range of people, professionals and partners. This includes local clinicians and professionals from across health and social care, including GPs. Patients, service users and unpaid carers, along with staff from the third sector and the independent sector, are also on the group. The membership is set out in the Act and associated guidance (you can read more here http://bit.ly/1LixouJ). The group will co-produce the strategic commissioning plan using their combined knowledge, expertise and experience. It will be informed by a Strategic Needs Assessment (SNA). Locality events Following events earlier in the year, another round of locality events was held in July in each of South Lanarkshire’s four localities. The events were attended by staff across health, social care, housing and voluntary and independent sectors. The agenda for the sessions focused primarily on an update on overall progress with integration, locality profiling, strategic commissioning and the forthcoming inspection of older people’s services. Joint inspection of services for older people in South Lanarkshire What is the inspection of older people’s services and why is it relevant as we integrate? The inspection is looking at our ability as a partnership - including health, social work, third and independent sector partners - to work together to achieve outcomes together. The outcome of the inspection will provide valuable feedback for the partnership in supporting older people and carers as we shape the strategic commissioning plan. What are we working towards? The H&SCP will be required to put in place arrangements to deliver all the services for which it is responsible. The strategic commissioning plan will set out how all the respective services will be delivered by the Spring of 2016. The more formalised integration processes build on an existing, well-established spirit of partnership working in South Lanarkshire.
