ROLE AND PURPOSE: The major responsibility of the Post Secondary Facilitator is to support teachers, students and parents in the development of Transition Plans as determined by State Performance Plan Indicator 13. QUALIFICATIONS: Education/Certification: • Bachelor’s Degree with teaching certificate in special education Experience: • Three years teaching experience in high school special education preferred MAJOR RESPONSIBILITIES AND DUTIES: I. INSTRUCTIONAL MANAGEMENT A. Arrange guest speakers for high school students regarding various supports. II.
STUDENT MANAGEMENT A. Conduct in-classroom presentations and trainings to students on the transition process including active and interactive website information. B. Assist students in completing applications and making post-secondary contacts. C. Assist in the development of the summary of performance for all graduating seniors. D. Follow-up on all graduated special education students.
III.
SCHOOL/COMMUNITY RELATIONS A. Provide, after-school hours, parent information sessions. B. Meet individually, as requested, with students and their parents to discuss transition needs.
IV.
PROFESSIONAL GROWTH AND DEVELOPMENT A. Provide support to special education teachers and support staff through training and modeling in the student information gathering process for the completion of transition assessments and transition IEP’s.
V.
MISCELLANEOUS A. Assist students, parents and teachers in the utilization of the AISD Transition Resource Guide and other available resources that facilitate movement toward the students’ postsecondary goals. B. All other duties assigned by the Director of Special Education.
