INSTRUCTIONS FOR COMPLETION OF APPLICATION FOR LICENCE TO PRACTISE AS A HEALTHCARE PROFESSIONAL IN THE KINGDOM OF BAHRAIN

Please read all the instructions carefully before completing any part of the application form. 1. Complete the form titled: “Application for a license to practise as a health professional” (form 1) according to the instructions. 2. Attach copies of the following items to your application (form1): a. Prospective employer letter confirming you have a job offer (for non-Bahrainis) b. One passport-size photo c. Pages of passport showing full name, DOB, date of issue and expiry of passport, nationality and sponsor’s name, if applicable. d. Statutory evidence of any name change, if applicable. e. General education certificate, secondary/high school (nursing/midwifery applicants only). f. Professional education certificate(s) (for all professions and specialties for which you are applying). g. Transcript of professional education, attested by Ministry of Foreign Affairs and Bahrain Embassy, in country of origin. h. Registration/license from all countries where you are currently licensed, if applicable. Note: Form 4 will be required from each authority (see 4.a. below). i. CV (from graduation to present, explaining any gaps in practice, e.g. study/unemployed) j. Bahrain CPR card / smart card. (See Note b. below). k. Health fitness certificate from health facility. Note: Certificate must indicate the applicant’s profession, to ensure the correct health assessment has been completed. (See Note b. below). l. Letter of good conduct from Ministry of the Interior (Bahraini Citizens only). NOTE: a. If any of the above documents are not in English or Arabic, certified attested translations from the Ministry of Foreign Affairs and Bahrain Embassy in your country of origin must be submitted with the document. [If you are residing in Bahrain, they can be attested by the local embassy of your country of origin and the Bahrain Ministry of Foreign Affairs.] b. Non-residents can submit items j & k after initial approval of application and/or arrival in Bahrain. c. Your application cannot be considered unless all the above attachments, in the correct format, are included in the application. Submission of complete applications in the correct format will avoid delays in the processing of your application. 3. Complete part I of the following forms: a. “Reference enquiry on professional practice” (form no 2) (two references required). Must provide references from direct supervisor. To avoid delays in your application, please note that other referees will not be accepted. b. Request for certification of professional education” (form no 3). Must be completed by either principal /registrar/chief administrative officer. Both forms and transcript must bear Effective Date: Janurary 2012

official original seals and stamp. (Not required if you obtained your professional degree in Bahrain.) c. “Request for verification of registration” (form no 4). Must be sent from every registration council/authority where you are or have been registered/licensed in the last five years.

NOTE: If you have never practised your profession, e.g. newly graduated for Bahraini & non Bahraini residents, you are not required to submit forms 2 or 4. 5. If you have practised in a GCC country, please provide your license and a reference (form 2) from that country. 6. Send forms 2, 3 and 4 to the concerned authority/school/employer for them to complete part II. Inform them to return the forms directly to the NHRA HCP Office. Please note that these forms will not be accepted unless submitted directly by the authority/school/reference. 7.
