Leading Health Care Reform B Y B U I L D I N G AC C O U N TA B L E C O M M U N I T I E S Southwestern Vermont Health Care, with program support from the Vermont Health Care Innovation Project, is delighted to present a regional health care conference on building accountable communities in this era of health care reform. Please join us on September 20th at the Grand Summit Resort Hotel and Conference Center at Mount Snow, VT, for an exciting day of learning and networking. A welcome reception at the resort is planned for the evening of September 19th at Harriman’s Restaurant. Our informative program features Mary D. Naylor, PhD, FAAN, RN, the Marian S. Ware Professor in Gerontology and Director of the New Courtland Center for Transitions and Health at the University of Pennsylvania School of Nursing. For the past two decades, Dr. Naylor has led an interdisciplinary program of research designed to improve the quality of care, decrease unnecessary hospitalizations and reduce health care costs for vulnerable community-based older adults. Plenary presentations will be made by Kevin C. Stone, BA, MBA, Project Specialist for Accountable Care Organization Development and Regional Services at Dartmouth-Hitchcock, and Heidi A. Klein, MSPH, Public Health Director of Planning and Health Care Quality for the Vermont Department of Health.

He has managed a consortium of home health and hospice agencies and a mobile imaging service. together clinicians, quality/safety improvement specialists, executives, community agency and services representatives, and leaders from across northern New England and beyond to share evidence on the development of accountable care communities that support population health.

■ Discussing,

comparing and contrasting most recent knowledge on the Transitional Care Model (TCM) and its application in rural, suburban and urban settings.

■ Developing

relationships among health care professionals in the region to further the aim of implementing best practices in integrated systems and developing partnerships that support improving population health and reducing medical costs while improving the individual’s experience.

■ P roviding

participants with an opportunity to learn how building an accountable community can strengthen and expand the Transitional Care Model to reach multiple high-risk patient populations and build strength through partnerships.

■ Sharing

a toolkit of information on how to replicate the successful work being done at Southwestern Vermont Health Care in order to assist others in building their own accountable community.

Hotel Arrangements: The Grand Summit Resort Hotel and Conference Center at Mount Snow will be the location for this conference. 89 Grand Summit Way, West Dover, VT 05356, or Mt. Snow Main Entrance, 394 Route 100, West Dover, VT 05356 A block of rooms is reserved at a special conference rate of $109 per room per night, plus 18% for tax and service fees. The block will be available until August 20, or until the blocked rooms are filled, at which time reservations will be subject to space and rate availability. To make your reservations and receive the conference rate, call 800-817-0764 and refer to the Southwestern Vermont Health Care group. Reservations for lodging must be made directly with the hotel.

Contact Hours: Southwestern Vermont Medical Center Education Department is an approved provider of continuing nursing education by the Vermont State Nurses Association's Committee on Education, an accredited approver by the American Nurses Credentialing Center's Commission on Accreditation. To receive credit for each session, conference participants will need to attend the entire session and complete an evaluation. This conference is approved for 4.25 nursing credit hours. It may take up to two months to receive your certificate for your CEUs.
