“I SURVIVED, NOW WHAT? TREATING THE MUSCULOSKELETAL CONSEQUENCES OF MATURING WITH A CHRONIC PEDIATRIC HEALTH CONDITION”

Matthew: Case Study illustrating purpose of the “I SURVIVED” course

Mary Massery, PT, DPT, DSc Owner, Massery Physical Therapy, Glenview, IL

Matthew: 1½ years old  

Matthew: Premature twin Multiple medical complications with resultant neurologic impairment Diagnosis: spastic quadriplegic cerebral palsy, numerous respiratory complications, poor feeder, poor talker

Collapsing forces from gravity on his spine and rib cage were compromising Matthew’s postural development and health status

Changes in postural alignment and postural control using musculoskeletal interventions

1 ½ y/o

4 y/o

Matthew 7 ½ y/o: Spinal management was a top priority and included rib and spinal mobilizations as well as dynamic bracing for head control and posture 

 

TLSO with large abdominal cutout for breath support was implemented by 4 y/o Usually worn with abdominal binder Goal: prevent or minimize spinal deformity to maximize health and participation

Matthew: 7½ years old

No spinal brace: Severe deforming forces on his developing spine

Spinal brace: Improved posture, improved dynamic extension of thoracic spine, improved head alignment, as well as other postural and functional improvements

Matthew

Without a focus on his musculoskeletal system, Matthew would likely develop a severe spinal deformity like David and be at high risk for respiratory complications

 In the course, the PT will learn musculoskeletal manual techniques and strategies to:  maximize proper spinal and rib cage maturation like Paul and Carl (both with cerebral palsy), and  minimize the risk for secondary respiratory complications.
