NOMINATE

Nominate a friend, neighbour or group for one of Bolton at Home’s Stars of the community awards. We’re hosting our Stars of the community awards and we’re looking for people who’ve made a difference and should be recognised for their efforts. It may be an individual or group who’ve worked to improve the lives of others, gone that extra mile, or made a significant contribution to their community. By nominating someone for one of our Stars of the community awards, you could help them win £50 to spend as they choose, a fabulous trophy and also attend Bolton at Home’s prestigious awards ceremony on Thursday 23 February 2017.

Categories Sparkling environment star

Stronger community star

We’re looking for individuals/ groups who’ve improved the local environment. Maybe you know somebody/a group who’s kept their garden/community garden blooming all year round. Maybe you know of, or have been involved in a community clean up campaign. Nominations could also be for projects that bring sustainability to a community such as food growing, recycling or ways to improve energy saving. We’re looking for projects that have not only improved the local environment but have also brought neighbours and communities together.

We’re looking for an individual or group who’s demonstrated drive and commitment to bring local people together to help build a stronger community. They’ll be known for what they do and be the driving force which supports activities for all sections of the community and has improved the skills and confidence of local people who have in turn gone onto achieve their own personal goals. All in all, this person or group will be going that ‘extra mile’ in order to give back to the community.

Outstanding achievement star

We’re looking to recognise a young person (under 25 years) who’s making a difference in their local community. Maybe you know a young person who, in their volunteering, brings people together, supports individuals or uses their skills and interests to help others.

We’re looking for an individual who’s shown motivation, commitment and passion for improving services in their local area in such a way that they’re seen as a community ambassador. This person will also show a continued commitment to making a ‘real difference’ in their neighbourhood.

Spirit of youth star

Personal achievement star We’re looking for an individual who’s grown in such a way and developed personally over the last 12 months. This could be by overcoming personal issues and showing a true commitment to making a change in their lives. This could also be a newcomer who’s taken the first steps towards getting involved and made a real impression on improving their community which has had a positive effect in their local area.

Healthy community star We’re looking for individuals or projects that have made a difference in helping tackle health and wellbeing in their community. Maybe there’s someone you know who’s started a food co-op, exercise class or provided drug and alcohol advice sessions or other projects which have addressed health and wellbeing, making a positive difference in the community.

Taking a stand star We’re looking for an individual or group who may have been a witness to anti-social behaviour and given evidence in court, set up a tenants or residents group to deal with noise or neighbour problems or organised events to prevent young people committing anti-social behaviour. This award looks to recognise the commitment and courage of local people who’ve played a key part in tackling anti-social behaviour.

Bolton at Home star We’re looking for a Bolton at Home staff member or team that have demonstrated outstanding customer service or who deserve to be recognised for their commitment and passion to make a positive difference to Bolton at Home customers lives. This person/ team may have ‘gone the extra mile’ to support a customer or may continually strive to provide excellent customer service in their day to day role. Please note: nominations for this category must come from Bolton at Home customers only.

NB: If you’d would like help with your nomination, then please don’t hesitate to contact the Community Investment Team or any other member of Bolton at Home staff at a housing office or UCAN centre.

How to enter You can nominate your stars by answering the questions and then:

• Completing this form and returning it FREEPOST to Stars of the community awards, Community Investment Team, Bolton at Home, FREEPOST RTES-CLYS-AYKR, 98 Waters Meeting Road, Bolton BL1 8SW. • Posting a DVD/CD/usb pen to the freepost address above with a covering note.

Rules of entry 1. Any customer of Bolton at Home, whether it’s an individual or group can nominate themselves or others. Bolton at Home staff members can also nominate for all categories except the Bolton at Home Star. 2. Nominees shouldn’t have received an award for the same activity in the previous 12 months. Nominations must be for new activity that’s taken place between Dec 2015 - Dec 2016. 3. Anyone nominated for the Spirit of youth award must be under 25 years old on 23 December 2016. 4. By attending the awards ceremony, nominees accept their names and images may be used in publicity both on and offline.

• Using our Facebook page at www.facebook.com/ fbboltonathome along with a covering post. • Emailing citeam@boltonathome. org.uk with a covering email. A maximum of six photos or two minutes of video can be submitted with a nomination. We strongly recommend you use video and photos to support your nomination as they will have a higher impact on the judges. If you need any help recording a supporting video clip, please call a member of the team on 01204 327922. It’s your responsibility to seek the consent of the nominees and any other individuals captured in the photo or film.

5. The judges’ decision is final. 6. The winners will be announced at our Stars of the community awards ceremony on Thursday 23 February 2017.
