NOMINATION FORM Nominate someone you know to be considered for a Young Black Achievers’ Award in one of the following categories: •

Outstanding Academic Achievement

•

Outstanding Achievement in Extra-curricular Activities

•

Outstanding Contribution to the Community

You’ll find more information about the Awards in this leaflet and a nomination form to compete on the back.

Closing date for nominations is 13 October 2017.

Who is organising these awards? Nottingham City Council’s Equality & Community Relations Team alongside IDEAL (Identity, Diversity and EAL).

Who is eligible for nomination? Children/young people of African, African-Caribbean and Black Mixed Heritage between the ages of 5 and 18 years who live in Nottingham City or are studying in Nottingham City schools/academies.

Who will be judging the competition? A panel of judges comprised of representatives from community-based organisations and local authority staff.

What happens next? Winning nominees will be notified by 27 October 2017. Certificates and prizes will be awarded in a presentation evening at the Council House on 9 November.

Can I nominate more than one person? There is no restriction on the number of nominations any individual may make, nor any restriction on the number of categories for which any individual may be nominated.

So how do I nominate someone? Please complete the nomination form accompanying this information and return it to the address shown on the form or forward it by email.

Guidance on completing the nomination form In order to give all nominees the best opportunity to receive one of the Young Black Achiever’s Awards, we have supplied guidelines below to support the completion of the nomination forms. (N.B. Primary schools can nominate previous Year 6 pupils who will be in Year 7 when the event takes place.) Please provide the following for:

1) All nominations • Approximately 200 words describing the nominee’s achievements, as well as the following:

2) For Academic Categories – “Outstanding Achievement in”... Nominations should contain some of the following information: • Academic level(s) achieved and progress made • SATs results • Number of GCSEs, grades and curriculum areas

3) Additional information This could include: • Sample/copy/evidence/recording of achievement • Mention of previously received awards and recognition

4) Young Person, Parent/Carer Contribution • Please inform the young person that you will be nominating them and allow them as well as their parent/carer to add a comment to the form.

All nominations should be submitted by 13 October 2017. Unfortunately, nominations received after this date cannot be considered. Winning nominees will be notified by 27 October 2017.
