Objectives The objectives of this meeting are as follows: • Understand the desired impact of providing countywide diabetes prevention and informational courses • Share knowledge about the needs of the residents with diabetes, pre-diabetes, or concerns about diabetes in Richland County

Reducing Disparities in Diabetes Care: Everyone with Diabetes Counts • •



Empower patients to effectively manage their chronic disease Improve clinical outcomes related to HbA1c, lipids, eye exams, weight, blood pressure control, and foot care Increase the number of opportunities for people to be supported in and learn how to self-manage their chronic disease

Our Goal The Richland Hospital is working with Richland County Health and Human Services and MetaStar, Inc., to develop a peer network in Richland County, Wisconsin. The network will offer a free educational Diabetes Empowerment Educational Program and community linkages/resources in order to reduce disparities for people at risk for diabetes by November 1, 2016.

Reaching Those at Risk for Diabetes in Rural Richland County • • •

Provide improved access to diabetes health counseling for people at risk for diabetes Extend the reach of health resources to rural Richland County Create links between existing resources
