What is the Palliative Care Service? The goal of our Palliative Care Service is to provide the best possible quality of life for people who are nearing the end of their life.

Where will I receive palliative care? You will stay in the palliative care unit, which has 12 beds—6 private and 3 semi-private rooms. We try to make your stay as comfortable as possible by creating a space that looks and feels like home. The patient rooms are decorated with soothing colours and art work. Each room has a bathroom, window, and a sleeper chair that folds out into a bed so family members and loved ones can stay overnight. For families, there is a dedicated shower on the unit, and a family lounge that has a television, fridge and microwave. Patients and families can also use the "quiet room" on the unit. The Bickle Centre is easily accessible by GO train, TTC, and is close to the highway so it is easy to get to by car.

What services are available to me? The palliative care service team will work with you and your family to meet your emotional, physical, and spiritual needs. Family and loved ones are welcome to visit at any time. We will help to support them as they share in your journey. We provide: • 24-hour care by health care professionals • Support to help you live and die with dignity • Pain and symptom control • Chiropody and dentistry services • Massage therapy by referral • Bedside television rental • Cafeteria • Social activities, for example: bingo, music hour, fine dining, smile theatre, personal pet visits, music hour, active games

Is the Palliative Care Service right for me? To be admitted you must: • Be 18 years of age or older • Be diagnosed with a life-limiting illness • Be expected to live for 3 months or less • Need a team of health care professionals to help you with your complex physical, medical or emotional needs • Need health care professionals to care for you 24 hours a day

Who is part of the palliative care team? As patients and family members you are very important members of the team. Palliative care patients will receive care and support from a team of health care professionals, including:  Palliative care doctor

 Occupational therapist

 Social worker

 Physiotherapist

 Wellness partners

 Speech language pathologist

 Registered nurses

 Respiratory therapist

 Registered practical nurses

 Spiritual care provider

 Dietitian

 Manager

 Pharmacist

What can I do to prepare for palliative care? What to bring: • Personal items such as clothing and toiletries • Personal pictures which can be placed on the bulletin board in your room Please do not bring large bulky items such as a dresser or a lazy boy chair.

What happens if I stay longer than 3 months? Palliative care is covered by OHIP for 3 months. Your health care team will talk with you about what may happen next. • The doctor may assess you again to see if you still need the support of the palliative unit • After careful planning with your health care team and before you move to another setting, you may qualify for a co-payment fee (an amount you pay depending on your income) • The team will talk with you about where you may go after your hospital stay and they can also support you to transition back to your home
