A vision of ‘good rehabilitation services’ 1. The patient, their expectations and outcomes are at the centre 2. The relationship between patient and provider is built upon joint responsibility and enables effective patientdirected care 3. Access to services is easy, open and includes self-referral. 4. The business model for delivery is framed on a clear definition, based on robust evidence, financial and risk management 5. Commissioners will procure clearly identified outcomes integrated around the individuals journey 6. The pathway is designed around the patient and coordinated to flow as a smooth journey across all sectors 7. The model of service offered is 7 days a week, multidisciplinary, defined by patient need, underpinned by safe practice and available to mixed groups of patients 8. The evidence base is robust with a focus on outcomes of rehabilitation and reablement, allowing for comorbidities and personalisation 9. The workforce a. utilises a broad range of personal and professional skills including those of service ‘navigators’, specialist professionals, reablement workers, carers, voluntary services and members of the community b. offers a breadth and depth of skills with extended scope of practice so that people’s needs are met flexibly with maximum quality of care c. provides professional leadership within teams to ensure quality standards of care whilst senior leadership from a breadth of 'rehabilitationists' at a policy level locally, regionally and nationally to ensure development of the rehab evidence base, and effective commissioning of quality services and the future workforce
