Process for Opening Jan Aushadhi Centre at CSC: Retail/Distributor

Retail  VLE who are interested to open Jan Aushadhi store have to apply for retail Jan Aushadhi centre by clicking on the link http://registration.csc.gov.in/JanAushadhi/default.aspx o Duly complete the form and submit it online.  Registration Fees of Rs 2000 will be deducted from the wallet of the VLE after completing the registration for retail Jan Aushadhi store on the above mentioned link.  After registration, the VLE will have to apply for the Drug Licence from their concerned State Drug Authority.  In case of retailer they have to apply for retail Drug Licence by filling up the form no 19, 19A and 19 C with other below mentioned documents o copy of commercial electricity bill, o land documents, o location Map, o copy of pharmacist details i.e copy of certificate, marksheets o Along with the DD of prescribed Fees. Please note that the licence should be in the name of “JAN AUSHADHI STORE”. That means the shop name on the licence should be “JAN AUSHADHI STORE” only.

 After getting the licence from concerned State Drug Authority, the VLE will have to send the scanned copy of the same to [email protected]  CSC SPV will send the same to BPPI requesting them to arrange the distribution of the drugs which the VLE has ordered from the product list provided by BPPI to the VLE CSC centre. o Note these products or the drugs have to be purchased by the VLE’s from BPPI. o The product list of available medicine is attached with this document  BPPI will provide marketing material (Banners and Pamphlets) for promotion of the CSC Jan Aushadhi store. 3|Page

Eligibilty criteria for Retail Jan Aushadhi Store: • Minimum required space of 120 sq. ft. • VLEs with either of the following qualification / VLEs with access to a pharmacist with the required background Diploma in Pharmacy (D.Pharm.)/ Bachelor of Pharmacy (B.Pharm)/ Master of Pharmacy (M. Pharm.) • Must procure a Retail drug license in the name of “JAN AUSHADHI” from the concerned State Drug Authority. • A registration Fees of Rs 2000 to be paid to BPPI (will be deducted from the wallet). • To Apply for Retail Store Submit the online registration form on the following link http://registration.csc.gov.in/JanAushadhi/default.aspx

Distributor  Registering for distributorship of Jan Aushadhi click on the link: http://csc.gov.in/new_newsletter/images/distributor_form.pdf o Duly complete the form and send the scanned copy along with society registration certificate to [email protected]  After registering the VLE society will have to apply for wholesale drug licence in the concerned State Drug Authority by filling the form no 19, 19C and 19 AA with all the below mentioned documents. o copy of commercial electricity bill, o land documents, location Map, o copy of pharmacist details i.e copy of certificate, marksheets o Along with the DD of prescribed Fees.
