Nomination Form Date: ____________ The Forest Park Elementary PTA Award Selection Committee requests that the members of the PTA and the school staff to assist in the selection of deserving recipients for recognition at our PTA Meeting on June 2nd, 2015 at the MPR. Nominated individuals who have made significant contributions to the wellbeing of children, families or activities in this school can be considered for this award. Current PTA members, School staff and teachers may also be considered for this award. You can nominate more than one person. Please use separate forms for each nomination. Nominations forms can be downloaded using the link http://bit.ly/1Fwfcea (the pdf is editable) Please see the attached information and guidelines for submitting the nominations. Instructions for Submitting the form: 1. Complete the form and place it in a sealed envelope. Write “Nominations: Ms. Clark” on the envelope. 2. Hand it to the class teacher or drop it off at the school office.

Information and Guidelines Deadline: Tuesday, May 19th, 2015 Awards Date: Date: Tuesday, 6/2/2015 Time: 7:00PM Venue: MPR

Awards Description:    

Honorary Service Award (HSA) – This award is given to recognize an individual for outstanding service to children and youth in your community, going above and beyond what is asked of them. Continuing Service Award (CSA) – This award is given to recognize an individual or organization for ongoing or long-time service to children and youth, providing support year after year. Volunteer of the Year Award (VYA) – This award is given to recognize an individual for their outstanding service to PTA. Golden Oak Service Award (GOSA) – This award is given to recognize, as the most prestigious of the Honorary Service Awards, an individual who has made significant contributions to the welfare of children in a school or community.

The committee will meet after May 22nd, open the envelopes and decide the winners. The envelopes will be under the care of Ms. Clark until then.

Guidelines: 1. You can submit nominations for more than one person. Please use a separate form for each nomination. 2. Person nominated can be one of these: PTA Member, PTA Board Member, School Staff, Teachers, Cross Walk Guards, Safety Patrol and Parents of kids studying in the school. 3. If you are a parent submitting a nomination, the name of your child, grade and classroom# are required. This will help us for tracking purposes. 4. Email address and Phone number are required in case we need to contact you for additional information. 5. Complete the form and place it in a sealed envelope. Write “Nominations: Ms. Clark” on the envelope. 6. Hand it to the class teacher or drop it off at the school office. 7. Results will be announced through flyers, FB and PTA Yahoo Groups.
