Each participant assumes responsibility for him or herself and on behalf of his/her executors, administrators and assigns, does fully and forever waive, release and discharge the Club’s owners, managers, employees and agents from any and all claims, demands, damages, rights of action or cause of action, present or future whether the same be known or unknowns, anticipated, resulting from, or arising out of, the Member or his/her guest, or his/her minor children’s use or intended use of the Club’s facilities and equipment. I consent to pictures being taken of the Participant and understand that any such pictures will become property of the Club. They may be used without payment of fees or other compensation to the Participant. • The NAC reserves the right to cancel or change the classes due to low enrollment. • Cancellations after the start of the session will not receive a refund or credit, unless a physicians’ note is provided with the request. • Participants who arrive more than 20 minutes late cannot be admitted to the class. • Please note: while we will endeavor to work with all children, any child who is disruptive to the class for more than five minutes will be removed. Credit will be issued to be used ONLY for future Rock Star classes. Where the Participant is a minor (under 18), a parent or legal guardian must sign below indicating agreement with the terms this application on behalf of the minor child.

RULES AND REGULATIONS • We request that all parents observe classes from the observation area located on the top deck that is overlooking the gym. • At the start of each class, please check in with your child’s coach/instructor. • Please be on time for class. No child can be admitted to the class more than 10 minutes late. • Take children to the bathroom prior to the start of their class. • Trampoline classes are an 1 and 30 minutes long. Please be on time to pick your child up from their class. • Classes with only one child in attendance will be reduced to 1 hour. • Please note: while we endeavor to work with all children, any child who is disruptive to the class for more than five minutes will be removed from the class. Credit will be issued to be used ONLY for futre Rock Star classes. • Rock Star makeup classes will only be offered for medical reasons with a note from a physician. Any makeup class must be arranged with the Rock Starz Director, Chris Lanctot.
