Please read the following before completing the application form. APPLICATION PROCEDURE 1) Please complete the application form and make sure you have attached all supporting documents before you submit.
2) Submit personally to: SMRT TAXIS PTE LTD Customer Service Centre - Level 3 60 Woodlands Industrial Park E4 S757705. NOTES FOR APPLICATION OF SMRT TAXIS & STRIDES BURSARY 2018 INTRODUCTION The purpose of this bursary is to provide financial assistance to children of our Partners, for the purpose of pursuing and completing their academic studies. ELIGIBILITY CRITERIA To apply for the SMRT TAXIS & STRIDES Bursary: You must be an active partner with us: Taxi/Strides hirer must be with us for at least 6 months prior to application date OR Taxi/Strides relief driver must be with us for at least 12 months prior to application date OR Taxi Share driver must be with us for at least 12 months and must complete an average of 150 booking hours with us for 3 months (July, August & September 2018). Verification of booking hours will be done backend and successful applicant will be notified by end October. Your child must be a student studying in Singapore and not be above the age of 25 years at the point of application. The Monthly Per Capita Income (PCI) must be below $1,500 per family member. Calculation of PCI: Monthly PCI =

CHECKLIST OF SUPPORTING DOCUMENTS 1) Proof of Relationship Photocopy of Birth Certificate of the Child Photocopy of NRIC of the parents / guardian living in the same household 2) Proof of Family Income (including Hirer / Relief) Photocopy of latest pay slip OR Income Tax Statement of all working family members living in the same household Photocopy of last 6 months’ CPF Statement of Account for non-working family members 3) Proof of Education Level for the Child Photocopy of the child’s Report Card / Result Slip / Letter of Acceptance from School BURSARY AMOUNT Each successful applicant will receive a one-time cash award of $500 per year per award. DISBURSEMENT PROCEDURE All applicants will be notified of the result of the application by end October 2018. Successful applicants will be required to attend the Bursary Award Ceremony in November/December 2018. IMPORTANT NOTES The full bursary awarded amount will be recovered from hirer/relief, if hirer/relief ceases hiring agreement with SMRT (withdrawal/termination) within 6 months after receiving the bursary award at the ceremony. 2
TERMS AND CONDITIONS OF SMRT TAXIS & STRIDES BURSARY 1) The application form has to be submitted personally to SMRT Taxis Pte Ltd, Customer Service Counter (Woodlands Depot) before 28 September 2018 5pm. 2) All applicants will be notified of the result by end October 2018. 3) Successful applicant will November/December 2018.

4) Each successful applicant will receive a one-time pay out of SGD $500 per year per award. 5) Our bursary is open to the following group of partners:
Taxi Partners (who have been an active hirer with us for at least 6 months or an active relief driver with us for at least 12 months), prior to the date of application.
Strides Partners (who have been an active hirer with us for at least 6 months or an active relief driver with us for at least 12 months), prior to the date of application.
Taxi Share Partners (who have been an active driver with us for at least 12 months and must complete an average of 150 booking hours with us for 3 months (July, August & September 2018). Verification of booking hours will be done backend and successful applicant will be notified by end October.
6) Successful applicant must remain in service with us for a period of at least 6 months from the date of the Bursary Award Ceremony. 7) The full bursary awarded amount will be recovered from the applicant, if the applicant ceases hiring agreement / stopped driving with SMRT (withdrawal/termination) within 6 months after receiving the bursary award. 8) The child must be a student studying in Singapore. Age limit is 25 years. 9) The Monthly Per Capita Income (PCI) must be below $1,500 per family member. 10) Our bursary is tenable for one year. A fresh application is required for every year. 11) Duplicate, incomplete or late submission will not be considered. 12) Our bursary is awarded based on financial needs. 13) Decisions made by the Bursary Committee are final.
