STAT INR Admit to ICU

INR > 1.4?

Yes

Give Vitamin K 10mg IV

Give NovoSeven IV rFVIIa Weight < 100kg = 1mg Weight > 100kg = 2mg

Transfuse thawed plasma 15ml/kg (4-6 units)

Yes

Recheck INR after 2, 4, 12, 24 hrs

INR > 1.4?

No
