VOLUNTEER REQUIREMENTS Volunteers must pay the volunteer fee of $25 prior June 1st 2014. All volunteers will receive a Four Winds invitational polo shirt and hat, as well as 1 round of golf to Blackthorn for their services. Food and beverages will be provided to volunteers each day. Volunteers must complete 9 total hours of shifts to receive their free round to Blackthorn. Volunteers must be at least 16 years of age prior to June 1st, 2014.

AGREEMENT AND SIGNATURE By submitting this application, I affirm that the facts set forth in it are true and complete. I understand that if I am accepted as a volunteer, any false statements, omissions, or other misrepresentations made by me on this application may result in my immediate dismissal. A $25 volunteer fee must be paid to Blackthorn Golf Club on or before June 1st, 2014.
